YOUR PRIVACY MATTERS
A thoughtful first step.
Our inquiry form is designed to collect the essentials, not your personal history.
What the form collects
Your name, email, optional phone number, preferred contact method, program of interest, and a short message. A phone number is required only when you ask to be contacted by phone. Counseling inquiries are limited to a brief contact request of 300 characters.
How inquiries are handled
Submissions are stored in private Lovable Cloud storage and are not publicly readable. Authorized organization managers can access inquiries through private management tools. Information is intended for responding to your request and discussing next steps using your selected contact method. Sending an inquiry is not a confirmed appointment or enrollment.
Keep sensitive information out
Do not send diagnoses, detailed trauma histories, medical records, financial information, or other sensitive records. A simple request to be contacted is sufficient. This form is not a clinical record system.
Your choices
Provide only a contact method that is safe for you. You may ask the organization to correct or delete your inquiry by sending a General Inquiry referencing your earlier submission. Do not include sensitive details in that request.
Important limits
No online communication can be guaranteed completely confidential. Private storage does not establish legal or clinical confidentiality. Retention periods, response times, practitioner qualifications, and any legal disclosure obligations must be confirmed directly with the organization before sharing further information.
If you need urgent help
This form is not monitored for emergencies. If you are in immediate danger, call your local emergency number. In the United States and Canada, call or text 988 for crisis support.
